Checklist of Requirements for Applicants

A. GENERAL REQUIREMENTS

  1. [ ] Duly accomplished application forms (fill in ALL entries)
  2. [ ] Recent (within the last 6 months) 2 x 2 colored pictures with white background (3)
  3. [ ] Diploma from a recognized medical school (certified true copy)
  4. [ ] PRC Board of Medicine certificate of licensure (certified true copy)
  5. [ ] Current PRC card (photocopy)
  6. [ ] Diploma from a PPS-accredited residency training program (certified true copy)
  7. [ ] Letter of intent
  8. [ ] Letters of recommendation (3)

    Note: The letters must attest to the applicant’s good moral & professional conduct, & location of pediatric practice. They must be composed by the recommending fellows themselves & not by the applicants.

    1. [ ]  Applicants with hospital base:
      1. [ ] NCR: from the department chair & 2 PPS fellows in good standing from the hospital.
      2. [ ] Non-NCR: from the chapter president & 2 PPS fellows in good standing from the hospital.
    2. [ ] Applicants without hospital base:
      1. [ ] NCR: from 3 PPS fellows in good standing from the area of practice.
      2. [ ] Non-NCR: from the chapter president & 2 PPS fellows in good standing from the area of practice.
  9. [ ] If the applicant has changed names, submit a copy of the marriage contract or any relevant supporting document. (certified true copy)

ADDITIONAL REQUIREMENTS FOR THE WRITTEN EXAMINATION

  1. [ ] Research paper completed within 5 years prior to the application & approved by the
    PPS Committee on Research Forum & Workshop (1 copy each)

    1. Authorship
      1. [ ] Senior author (1 research paper)
      2. [ ] Co-author (2 researches). Co-authorship must be confirmed by
        the primary author through a letter addressed to the secretary of the PPS
    2. Publication
      1. [ ] Published
      2. [ ] Non-published – approved by the PPS Committee on Research Forum
        & Workshop (attach approval sheet)
  2. [ ] Must have taken the PPS WISE (Written In-Service Examination) TWICE since the
    start of residency training (certificate to be issued by the WISE secretary)
  3. [ ] Certificates of attendance with CPD units: minimum of 25 units per year excluding
    the PPS convention (if the examination will be taken more than a year after the
    completion  of residency)
  4. [ ] Non-refundable examination fee upon submission of the application

ADDITIONAL REQUIREMENTS FOR THE  ORAL EXAMINATION

  1. [ ] Must have passed the written examination
    1. [ ] Level I, II & III – at least 2 years before the oral examination
    2. [ ] Chief Resident in a Level III program – at least 1 year before the oral examination
    3. [ ] Level 4 – anytime after passing the written examination
  2. [ ] Certificates of attendance with CPD units: minimum of 25 units per year excluding the PPS convention, since passing the written examination.

    Note: An active faculty in Pediatrics in a medical school shall be given CPD units equivalent to this requirement.

  3. [ ] Any 1 of the following:
    1. [ ] Pediatric practice
      1. [ ] Graduates of Levels I, II & III programs – 2 years
      2. [ ] Graduates of Level IV programs – 1 year
      3. [ ] Chief residents of Level III programs – 1 year
    2.  [ ] Two years of fellowship training in a subspecialty accredited by the respective
      subspecialty board
    3.  [ ] Chief resident of Level IV programs
    4.  [ ] Master’s degree related to Pediatrics
    5.  [ ] Full time faculty in Pediatrics for at least 2 years in a recognized medical school
  4. [ ] Non-refundable examination fee upon submission of the application

REQUIREMENTS FOR RECLASSIFICATION TO FELLOW

  1. [ ] General requirements
  2. [ ] Letter of application
  3. [ ] Certificate as DIPLOMATE (photocopy)
  4. [ ] Pediatric practice – any of the following:
    1. Ten years as DIPLOMATE IN GOOD STANDING,
    2. Five years as DIPLOMATE IN GOOD STANDING AND research completed AFTER certification as diplomate either approved by the PPS Committee on Research Forum &
      Workshop (attach approval sheet), presented in the PPS Convention, or published, as:

      1. Senior author (1 research)
      2. Co-author (2 researches). Co-authorship must be confirmed by the primary author through a letter addressed to the secretary of the PPS
  5. [ ] Certificates of attendance with CPD units: minimum of 25 units per year excluding the PPS convention, since passing the  written examination.

    NOTE: An active faculty in Pediatrics in a medical school shall be given CPD units equivalent to this requirement

  6. [ ] Certificate of post-residency training, if any
  7. [ ] Non-refundable processing fee upon submission of the application

REQUIREMENTS FOR RECLASSIFICATION TO EMERITUS FELLOW

  1. [ ] Age – 65 years old at the time of application
  2. [ ] Birth certificate (certified true copy)
  3. [ ] FELLOW IN GOOD STANDING for at least 20 years
  4. [ ] Duly accomplished application forms (fill in ALL entries)
  5. [ ] Letter of intent
  6. [ ] Letters of recommendation from:
    1. [ ] Two PPS fellows in good standing from the place of practice
    2. [ ] Chapter president (if the place of practice is outside NCR)
  7. [ ] Certificate as FELLOW (photocopy)
  8. [ ] Recent (within the last 6 months) 2 x 2 colored pictures with white background (1)
  9. [ ] Non-refundable processing fee upon submission of the application

REQUIREMENTS FOR HONORARY FELLOW

  1. [ ] Letter of nomination from a PPS FELLOW IN GOOD STANDING
  2. [ ] Fellow of an internationally recognized pediatric specialty board
  3. [ ] Academic rank of FULL PROFESSOR
  4. [ ] International prestige
  5. [ ] Significant contribution to the professional advancement of the PPS
  6. [ ] Unanimous approval by the Specialty Board with the concurrence of the
    majority of the Board of Trustees

REQUIREMENTS FOR AFFILIATE FELLOW

  1. [ ] Letters of nomination from a PPS FELLOW IN GOOD STANDING (2)
  2. [ ] FELLOW IN GOOD STANDING of a specialty related to pediatrics
  3. [ ] Practice
    1. At least 5 years AND submission of published research as senior author, OR
    2. At least 10 years
  4. [ ] Duly accomplished application forms (fill in ALL entries)
  5. [ ] Recent (within the last 6 months) 2 x 2 colored pictures with white background (3)
  6. [ ] Letter of intent
  7. [ ] Should pay annual dues to maintain status in good standing

REQUIREMENTS FOR CORRESPONDING MEMBER

Note: A corresponding member is any member who resides abroad & desires to continue his membership.

REQUIREMENTS FOR NON-RESIDENT DIPLOMATE

 

  1. [ ] Diploma from a recognized medical school in the country of origin (certified true copy)
  2. [ ] License to practice medicine, or its equivalent, in the country of origin
  3. [ ] Diploma from a PPS-accredited residency training program (certified true copy)
  4. [ ] Should have passed the written & oral examinations of the SB
  5. [ ] Duly accomplished application forms (fill in ALL entries)
  6. [ ] Letter of intent
  7. [ ] Recent (within the last 6 months) 2 x 2 colored pictures with white background (1)
  8. [ ] Should pay annual dues to maintain status in good standing
